Exosome therapy shows promise as a minimally invasive treatment for androgenetic alopecia (AGA), alopecia areata (AA), and chemotherapy-induced alopecia (CIA), with strong support from preclinical studies. However, clinical data are still preliminary and varied. To advance exosome therapy into routine clinical practice, there is a need for large-scale, multicenter randomized trials with standardized outcomes. Additionally, harmonized manufacturing and regulatory frameworks, including GMP-compliant production and batch-level quality control, are essential to ensure the reproducibility and safety of this therapy.
