Feminizing Adrenocortical Carcinoma Without Gynecomastia

    TLDR A man with a rare adrenal cancer showed unusual symptoms and died four months after diagnosis.
    The document described a rare case of feminizing adrenocortical carcinoma in a 44-year-old man who presented with abdominal pain and a large retroperitoneal tumor with numerous metastases. Despite high levels of estrogen and other adrenal hormones, the patient did not exhibit gynecomastia, which is typically a major symptom. The absence of gynecomastia was hypothesized to be due to resistance of breast tissue to estradiol. The patient underwent surgery and chemotherapy but unfortunately died four months after diagnosis. The prognosis for such tumors is generally poor, especially when metastases are present at diagnosis.
