Preservation of Fertility in Transgender Men on Long-Term Testosterone Therapy: A Systematic Review of Oocyte Retrieval Outcomes During and After Exogenous Androgen Exposure

    TLDR Oocyte retrieval is possible for transgender men on testosterone therapy without affecting fertility outcomes.
    This systematic review investigated the feasibility and outcomes of oocyte retrieval in transgender men undergoing testosterone therapy. It included 7 case reports, 4 cohort studies, and 2 cross-sectional studies. The findings indicated that oocyte retrieval was viable after prolonged androgen exposure, and there was no direct relationship between the duration of testosterone suspension and fertility preservation outcomes. The review suggested that continuous testosterone administration and the use of aromatase inhibitors during ovarian stimulation could mitigate the distress associated with hormonal ovulation induction. The study highlighted the need for further exploration of fertility preservation methods that align with reproductive desires while minimizing gender dysphoria from hormone therapy suspension.
