    TLDR Epiduo® is an effective first-line acne treatment that reduces inflammation and combats antibiotic resistance.
    Acne vulgaris involved multiple abnormalities, including hyperkeratinization, increased sebum production, bacterial proliferation, and inflammation. Recent studies indicated that inflammation preceded skin lesions, with Th17 lymphocytes and inflammasome playing significant roles. Adapalene 0.1%/BPO 2.5% (Epiduo®) was highlighted as an effective treatment, addressing anti-comedogenic, anti-inflammatory, and anti-bacterial factors, and endorsed by global guidelines. Despite this, many clinicians continued using topical antibiotics, leading to increased antimicrobial resistance. Studies showed that Epiduo® effectively inhibited antibiotic-resistant P. acnes strains and was recommended as a first-line treatment. The combination of adapalene and BPO significantly decreased markers of proliferation, adhesion, differentiation, and innate immunity in inflammatory acne skin, explaining its comedolytic and anti-inflammatory activities. Epiduo® and doxycycline were also found to be effective alternatives to oral isotretinoin for severe acne.
