Non-medical androgen use among recreational gym-goers poses significant health risks and substance dependence issues, necessitating an integrated medical and harm-reduction approach in primary care settings. Early detection of cardiovascular risk factors is essential to address reversible complications. The case report emphasizes the need for reassessing legal and clinical measures to enhance harm reduction and care provision. Despite adherence to best practices and a year of abstinence, the patient's fatal outcome highlights the urgent need for further research, increased awareness, and stronger prevention and harm reduction strategies for individuals affected by non-medical androgen use.
