TLDR High-dose testosterone with finasteride improves muscle health in males with spinal cord injury.
This editorial highlights eight recent studies on muscle atrophy, focusing on therapeutic interventions, diagnostic biomarkers, and potential molecular mechanisms. The studies employ various research designs, including randomized controlled trials, meta-analyses, and case-control studies. Notably, a pilot study by Otzel et al. found that high-dose testosterone replacement therapy combined with finasteride significantly improves body composition and musculoskeletal health in males with chronic incomplete spinal cord injury. The research underscores the multifactorial pathogenesis of muscle atrophy and suggests new directions for clinical management, emphasizing personalized and multi-targeted strategies. Despite advancements, challenges such as limited sample sizes and methodological heterogeneity persist, highlighting the need for longitudinal studies to better understand sarcopenia progression.
