The study demonstrated that a biofunctional hydrogel, incorporating mesenchymal stem cell-derived small extracellular vesicles (sEVs) into an injectable hyaluronic acid hydrogel, promoted scarless skin healing in a mouse large skin injury model. This hydrogel facilitated wound healing by modulating immune responses, specifically by driving macrophages towards an anti-inflammatory and anti-fibrotic (M2c) phenotype. This shift inhibited fibroblast activation, which is crucial for preventing fibrotic scar formation. The findings suggested that immune response modulation could be an effective strategy for achieving scarless skin wound healing.
