Overcoming Hurdles: Side Effects with Old Drugs and Achieving Success with Novel Drug in Alopecia Areata

    An 11-year-old girl with alopecia totalis experienced complete hair loss on her scalp and was initially treated with topical minoxidil, azathioprine, and dexamethasone. The treatment led to hair growth on her forehead but not on her scalp, and azathioprine caused transaminitis. After discontinuing azathioprine and minoxidil, and starting tofacitinib, the girl's scalp hair regrew, and forehead hypertrichosis resolved. The case highlights the importance of proper minoxidil application to avoid hypertrichosis and demonstrates the effectiveness of tofacitinib in treating alopecia areata. Proper patient counseling on minoxidil use is crucial to prevent side effects.
