Comparison of Resumption of Ovulation After Cessation of Oral Contraceptives and Medroxyprogesterone Acetate in Women With Polycystic Ovary Syndrome

TLDR Medroxyprogesterone acetate is as effective as oral contraceptives for resuming ovulation in women with PCOS.
This study evaluated the resumption of ovulation in 272 women with polycystic ovary syndrome (PCOS) after stopping oral contraceptive pills (OCPs) or cyclic medroxyprogesterone acetate (MPA). Ovulation resumed in 18.4% of women after stopping MPA and in 24.3% after stopping OCPs, with similar odds of ovulation resumption between the two groups (adjusted odds ratio of 1.00). The study concludes that cyclic MPA is a viable alternative to OCPs for managing menstrual disturbances in women with PCOS, particularly for those who cannot or choose not to use OCPs. Despite being retrospective and conducted at a single center, the study used robust methods to adjust for confounders and ensure accurate ovulation assessment.
