This systematic review highlights a strong association between oxidative stress and the severity of alopecia areata (AA), as evidenced by elevated oxidant and reduced antioxidant biomarkers correlating with higher SALT scores. The review included 10 studies from 8 countries, with participants averaging 29.9–38.1 years old and 51.3% male. Malondialdehyde (MDA) was the most frequently evaluated biomarker. The findings underscore the need for standardized, large-scale prospective studies to clinically validate these associations and improve the assessment of AA severity.
