Pyoderma Gangrenosum Presented as Generalized Pustules in a Patient With Crohn's Disease

A 21-year-old male with Crohn's disease and pyoderma gangrenosum (PG) presented with generalized pustules, leg ulcers, and scalp hair loss. The case highlights the importance of recognizing atypical manifestations of inflammatory bowel disease (IBD), as PG can mimic infections and lead to misdiagnosis. The patient was treated with a combination of intravenous methylprednisolone, oral prednisolone, mesalamine, azathioprine, isotretinoin, and infliximab, resulting in significant improvement, including re-epithelialization of leg ulcers and substantial hair regrowth. This case underscores the need for accurate diagnosis and timely management of extraintestinal manifestations of IBD.
