TLDR Plant-based compounds may offer safer prostate cancer treatment with fewer side effects.
The review discussed the potential of phytochemicals, such as phytosterols, polyphenols, and fatty acids, as natural alternatives to synthetic 5-alpha-reductase inhibitors (5αRis) for prostate cancer treatment. Synthetic 5αRis like finasteride and dutasteride were effective but had adverse side effects, leading to interest in plant-derived compounds. Phytosterols like β-sitosterol and stigmasterol, and polyphenols like quercetin, myricetin, and fisetin, showed promise in inhibiting 5αR activity and reducing prostate tumor growth. Catechins from green tea also demonstrated potential as 5αR inhibitors. These findings suggested that phytochemicals could serve as complementary or alternative treatments for prostate cancer with fewer side effects than conventional medicines, highlighting the need for further research into these plant-based compounds.

FCE 28260 (PNU 156765), an under-explored 5α-reductase inhibitor, showcases promising results in research by Giudici et al., outperforming well-known treatments like Finasteride in reducing the conversion of testosterone to DHT. Its superior efficacy, demonstrated through lower IC50 values in both natural and human recombinant enzyme studies, suggests it could offer more effective management of DHT-related conditions. Additionally, its lower molecular weight hints at better potential for topical application, potentially offering advantages in treating conditions such as androgenic alopecia. Despite its potential, it has not advanced in development, possibly due to financial limitations, leaving its therapeutic prospects and side effect profile largely unexplored.
