Leydig cell tumors (LCT) of the ovary are extremely rare, accounting for less than 0.1% of all ovarian tumors, and are typically associated with symptoms of excess androgen levels. This case study discusses a 35-year-old pregnant woman who was found to have a 9 cm unilateral ovarian cystic mass incidentally during a cesarean section. Following the delivery of a healthy male baby, a left oophorectomy was performed, and the mass was identified as a pure Leydig cell tumor based on morphological and immunophenotypic findings. The study concludes that while LCTs are uncommon, they can present with a range of symptoms from rapid virilization to being asymptomatic.
