Step-Down Approach to Concentration of Intralesional Triamcinolone Acetonide in Treatment of Scalp Patchy Alopecia Areata: Insights from an Open-Label Randomized Trial

    This study assessed the effectiveness and safety of two concentrations of intralesional triamcinolone acetonide (TA) in treating scalp patchy alopecia areata in 59 patients. Group A received 5 mg/ml and Group B received 10 mg/ml of TA every 3 weeks for up to 4 sessions. Group B experienced significantly faster hair regrowth, achieving a regrowth scale (RGS) score of 4 (>75% hair regrowth) more quickly than Group A. By 12 weeks, both groups showed comparable efficacy and side effects. The study suggests considering a "step-down" or "step-up" approach with patients to balance faster response with potential side effects.
