    TLDR Testosterone creams and gels can be effective but have serious risks and should be used only with medical supervision.
    This systematic review evaluates the composition, mechanisms, efficacy, safety, and risks of topical testosterone formulations, such as creams, gels, ointments, and transdermal patches, used primarily by middle-aged and elderly men. The review, covering studies from 2010 to 2025, finds that while testosterone creams have variable absorption, gels provide stable bioavailability, and patches ensure continuous release. Despite their effectiveness in increasing serum testosterone levels, these formulations can lead to adverse effects like acne, gynecomastia, alopecia, and potential stimulation of prostate or breast cells. The study concludes that due to significant systemic risks, these products should only be used under strict medical supervision.
