Successful Treatment of Coexisting Alopecia Areata and Bullous Pemphigoid With Dupilumab: A Case Report

    TLDR Dupilumab successfully treated a woman's alopecia areata and bullous pemphigoid, leading to full hair regrowth and symptom resolution.
    This case report describes a 71-year-old woman with coexisting alopecia areata (AA) and bullous pemphigoid (BP), who showed significant improvement after treatment with dupilumab, an IL-4Rα antagonist. Despite being refractory to multiple therapies, including corticosteroids and topical treatments, the patient experienced complete hair regrowth and resolution of BP after one year on dupilumab, and was able to discontinue treatment without relapse. This case highlights the potential of dupilumab in treating autoimmune conditions with overlapping Th1 and Th2 inflammatory pathways, suggesting a need for further research into individualized immunomodulatory therapies for complex dermatologic diseases.
