This review addresses chemotherapy-induced alopecia (CIA), highlighting its psychological impact and the emerging issue of persistent CIA (pCIA). It examines the condition's epidemiology, psychosocial effects, and the damage to follicular stem cells caused by taxane and alkylating agents. The review evaluates current prevention and treatment methods, such as scalp cooling, minoxidil, prostaglandin analogues, antiandrogens, low-level light therapy, platelet-rich plasma, and hair transplantation. A clinical algorithm is proposed for managing CIA during and after chemotherapy. The review advocates for future strategies that incorporate molecular risk assessment, patient feedback, and a multidisciplinary approach to make CIA a preventable and manageable condition.
