Vascular, Neurologic and Hormonal Abnormalities in Men with Persistent Sexual Dysfunction After Discontinuation of Finasteride

    TLDR Men may experience long-term sexual dysfunction after stopping Finasteride.
    This study aimed to replicate previous findings on persistent sexual dysfunction in men after discontinuing Finasteride (FIN). Reviewing charts from 2015-2020, 91 men (median age 39) with normal sexual function prior to FIN use and persistent symptoms for over 6 months post-discontinuation were analyzed. The most common symptom was erectile dysfunction (ED), present in 95% of cases, with 77% showing abnormal erectile tissue inhomogeneity on ultrasound. Additionally, 57% experienced orgasmic dysfunction, and 48% had genital anhedonia/anesthesia. Quantitative Sensory Testing (QST) revealed abnormal results in 60% of patients. Hormonal abnormalities included low dihydrotestosterone (DHT) in 30%, low testosterone (T) in 16%, and low calculated free T in 9%. The study confirms persistent physical sequelae in this population, highlighting the need for further research.
