November 2020 in “JAAD Case Reports” This study explored the use of the Janus kinase (JAK) inhibitor tofacitinib in treating patients with both alopecia areata (AA) and inflammatory bowel disease (IBD), specifically ulcerative colitis (UC) and Crohn disease. A retrospective chart review was conducted on 9 patients aged 18 and older, revealing that 6 out of 7 patients with active IBD achieved remission of both IBD and AA with tofacitinib. The study highlighted the potential overlap in the pathobiology of AA and IBD and suggested that JAK inhibitors could be a viable treatment option for patients with these comorbid autoimmune conditions. However, the study was limited by its small sample size and the fact that all patients were from a single clinic.

This indicated that baricitinib could mitigate inflammatory responses in AA. The research underscored the potential of JAK inhibitors in addressing mitochondrial and oxidative stress-related mechanisms in AA pathogenesis, suggesting their benefit in managing keratinocyte inflammation.
