This case report highlights the successful use of upadacitinib, a selective JAK1 inhibitor, in treating a 57-year-old man with severe atopic dermatitis, vitiligo, and alopecia areata, conditions that share the JAK-STAT signaling pathway. Conventional therapies had failed, but upadacitinib (15 mg daily) led to rapid improvement: pruritus subsided within days, AD-related erythema cleared by 15 weeks, complete eyebrow regrowth occurred within months, and significant vitiligo repigmentation was observed at one year. The treatment was well-tolerated, suggesting that upadacitinib monotherapy could be an effective and well-tolerated option for these co-existing conditions, supporting a "multi-disease therapy" approach targeting common pathogenic pathways.
