A 35-year-old woman experienced pain and patches of nonscarring alopecia with erythema and edema after receiving a cosmetic procedure involving poly-L-lactic acid (PLLA) on her face and frontal hairline. The diagnosis was confirmed through trichoscopy and histopathologic analysis. The report highlights the importance for dermatologists to recognize the potential for embolization and retinal ischemia associated with PLLA use. Early recognition and management of this rare adverse event can be improved by combining trichoscopy with histopathologic analysis.
