    This narrative review addresses the management of androgenetic alopecia (AGA) in transgender and gender-diverse (TGD) individuals, emphasizing the need for personalized treatment plans that align with both hair regrowth and gender-affirming goals. While treatments like topical and oral minoxidil and 5-alpha reductase inhibitors show promise, there is a lack of large studies specifically focused on TGD patients. The review highlights the use of spironolactone and bicalutamide for feminization in transfeminine individuals, with a caution for liver function monitoring. Nonpharmacological therapies and gender-affirming procedures, such as hair transplantation, are recommended for their psychological benefits, though specific studies in TGD populations are limited. The review calls for future research to validate these approaches, as current recommendations rely on data from cisgender populations and expert opinion.
