Frontal fibrosing alopecia (FFA) is a common form of scarring alopecia, primarily affecting postmenopausal women, characterized by progressive hairline recession and often accompanied by eyebrow and body hair loss. While its pathogenesis is not fully understood, hormonal, genetic, autoimmune, and environmental factors are considered contributors. The role of cosmeceuticals, particularly sunscreens and facial moisturizers, has gained attention as potential environmental triggers, with patch testing revealing sensitization to allergens in these products. However, the link between cosmeceuticals and FFA remains controversial due to limitations in existing studies. As FFA prevalence rises, further research into environmental triggers is crucial.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
