Effects of Mitotane Treatment on Human Steroid Metabolism: Implications for Patient Management

    The study on mitotane treatment in 17 patients with adrenocortical carcinoma (ACC) revealed significant alterations in cortisol metabolism, including increased excretion of polar unconjugated cortisol metabolites and decreased common cortisol metabolites, due to specific inhibition of 5α-reductase type 2. These changes necessitated higher hydrocortisone doses during treatment and suggested mitotane's potential as a probe for steroid metabolism. Despite these metabolic effects, they did not interfere with early ACC recurrence detection nor served as a surrogate for monitoring mitotane's therapeutic efficacy. The study also noted that mitotane's induction of CYP3A4 could affect the bioavailability of other drugs.
