Elevated serum BHB levels were observed in AA patients and correlated with the severity of hair loss. BHB treatment increased inflammatory cytokine production in outer root sheath cells, suggesting that high BHB levels may worsen the inflammatory response in AA, leading to chronic hair loss and treatment resistance. The findings indicate that serum BHB levels could be a marker of poor prognosis in severe AA cases, highlighting the need for further research to understand BHB's role in AA pathogenesis and management.
