A phase 3 trial involving 330 adults with severe alopecia areata demonstrated that ivarmacitinib, a selective JAK1 inhibitor, significantly improved hair regrowth with a favorable safety profile. At week 24, 34.9% of participants in the 4 mg group and 40.6% in the 8 mg group achieved a SALT score of 20 or less, indicating at least 80% scalp hair coverage, compared to 9% in the placebo group. The treatment effect was statistically significant, with a 25.6% improvement for the 4 mg dose and 31.6% for the 8 mg dose over placebo. By week 52, those initially on placebo who switched to ivarmacitinib showed further improvement, with 47% in the 4 mg group and 63% in the 8 mg group achieving SALT20. The safety profile was consistent across groups, with no new safety concerns reported. These findings suggest ivarmacitinib as a promising and well-tolerated treatment option for severe alopecia areata.
