This case report detailed a 31-year-old woman with a rare ovarian steroid cell tumor not otherwise specified (NOS), who presented with secondary amenorrhea, hirsutism, and infertility. Following a left salpingo-oophorectomy, her hormone levels normalized, menstruation resumed, and she conceived spontaneously 12 months later, delivering a healthy baby. The report emphasized that these tumors are often benign but can cause virilizing symptoms, with elevated 17α-hydroxyprogesterone levels being common. Surgical removal was crucial to prevent irreversible virilization. This case contributed to the limited data on fertility outcomes post-surgery for such tumors, with only two other similar cases documented.
