Case Report: Dupilumab Induced Sustained Complete Remission in a Child With Severe Alopecia Areata and Markedly Elevated IgE

TLDR Dupilumab effectively treated severe alopecia areata in a child.
This case report describes a 7-year-old girl with severe alopecia areata and high IgE levels who achieved complete remission with dupilumab treatment. Despite previous ineffective glucocorticoid therapy, the patient showed significant hair regrowth after 8 weeks of dupilumab, with a SALT score reduction from 95 to 0 by week 48. Her IgE levels also decreased from 2300 IU/mL to 330 IU/mL. The treatment was well-tolerated, with only a mild allergic reaction after the first injection. This case suggests dupilumab as an effective treatment for severe pediatric alopecia areata with high IgE, highlighting the potential of IgE monitoring and dosing interval extension in maintenance therapy.
