Case Report: Dupilumab Induced Sustained Complete Remission in a Child With Severe Alopecia Areata and Markedly Elevated IgE

TLDR Dupilumab effectively treated severe alopecia areata in a child.
A 7-year-old girl with severe alopecia areata and high IgE levels achieved complete remission after 48 weeks of treatment with dupilumab, a monoclonal antibody targeting IL-4Rα. The treatment resulted in significant hair regrowth, reducing her SALT score from 95 to 0, and decreased her serum IgE from 2300 IU/mL to 330 IU/mL. Initially administered every 4 weeks, the treatment interval was extended to every 6 weeks after improvement, maintaining its efficacy. This case suggests dupilumab as a promising treatment for severe pediatric alopecia areata, particularly in patients with elevated IgE, and highlights IgE's potential as a biomarker for treatment response.
