Using multi-omics approaches, 146 MSCs-derived genes were initially identified, with KRT6A emerging as the most critical candidate. KRT6A was found to be a robust diagnostic marker and a predictor of disease severity, with significant negative correlation to immune dysregulation in AA. It is primarily enriched in skin hair follicle tissues and is downregulated in AA models. Experimental validation showed that KRT6A-overexpressing human hair follicle mesenchymal stem cells (HFMSCs) enhanced cell migration and alleviated alopecia in AA mice, while knockdown reversed these effects. This suggests that KRT6A could be a promising target for developing targeted therapies for AA.
