A 47-year-old male presented with a giant, atypical proliferating pilomatrixoma on his left upper eyelid, measuring 7 × 10 cm. The lesion was surgically removed, and histopathological examination confirmed it as an atypical pilomatrixoma with minimal infiltrating margins and focal cytological atypia. No recurrence was observed one year after surgery. The study highlights the importance of excising enlarging periocular masses for histopathological evaluation to rule out malignancy, as pilomatrixoma can mimic other lesions and has potential for malignant transformation.
