A 7-month-old mixed breed dog with dermatitis caused by Malassezia fungal infection and Rhipicephalus sanguineus tick infestation showed symptoms such as itching, alopecia, and oily, rancid skin. Diagnosis was confirmed through cytological examination and the dog exhibited lymphocytosis and hyperchromic macrocytic anemia. Treatment included oral itraconazole, loratadine, fish oil, sebasol shampoo, and multivitamins. After 14 days, the dog's condition improved with reduced symptoms and hair regrowth. To prevent recurrence, it is advised to keep the dog in a dry environment, maintain cleanliness, and limit contact with stray dogs.

A 30-year-old woman in Pakistan presented with symptoms including intermittent fever, joint pains, oral ulcers, photosensitivity, weight loss, hair fall, dry cough, and shortness of breath. Examination revealed malar hyperpigmentation, alopecia, nail dystrophy, erythema, and multiple skin ulcers. She also had synovitis in several joints. Imaging showed bilateral ground-glass opacification, and lab tests indicated positive Anti-Nuclear Antibody and Anti-Ro antibody, with reduced serum complement levels. She was diagnosed with Lupus Vasculitis and treated with steroids, mycophenolate mofetil, and hydroxychloroquine.

The study conducted on 1,000 patients in South Asia highlights that the misuse of topical steroids has significantly altered the clinical presentation of dermatophytosis, leading to atypical, extensive, chronic, and recurrent infections. The most common atypical morphologies observed were ill-defined erythematous plaques, extensive concentric lesions, and pseudo eczematous plaques. The study found a significant association between prolonged steroid use and the severity of the disease. The findings underscore the need for regulatory control of steroid combinations, public education, and increased clinician awareness to address the growing issue of steroid-modified dermatophytosis.
