Over 12 weeks, significant improvements were observed: hair density increased by 44.4 hairs/cm², the proportion of vellus hairs decreased, hair shaft diameter increased by 22.4 µm, and the proportion of anagen hairs rose by 36.4%. Histological analysis showed increased follicular density and normalization of the terminal/vellus ratio, while immunohistochemistry indicated enhanced cellular proliferation and angiogenesis. No serious adverse events were reported. The treatment effectively restored hair follicle function and reversed miniaturization, suggesting salmon-derived exosomes as a promising regenerative strategy for androgenetic alopecia.
