November 2023 in “Journal of Investigative Dermatology” This post-hoc analysis examined the response patterns of 191 patients with alopecia areata (AA) treated with ritlecitinib in the ALLEGRO phase 2b/3 and ALLEGRO-LT studies. Patients aged ≥12 years with ≥50% hair loss were categorized based on their Severity of Alopecia Tool (SALT) score trajectories over 96 weeks. Results showed that 45.5% of patients were responders, with 18.3% responding early, 16.8% in the middle period, and 10.5% late. Additionally, 12.6% were partial responders, 29.3% were non-responders, and 12.6% were relapsers. Factors such as female sex, younger age, absence of active shedding, shorter duration of scalp hair loss, and lower baseline SALT score were linked to a higher likelihood of response. The study suggests that over 45% of patients benefit from ritlecitinib, with most responding within 1 year, although some may require extended treatment.
