Immune Therapies for Alopecia Areata: Evidence and New Perspectives

    Alopecia areata (AA) is an autoimmune condition causing patchy hair loss due to immune attacks on hair follicles. Current treatments like corticosteroids and immunosuppressants have variable effectiveness and side effects. Recent research highlights JAK inhibitors and biologics targeting specific immune pathways as promising new therapies. JAK inhibitors, such as tofacitinib, ruxolitinib, and baricitinib, have shown efficacy but may cause side effects like increased infection risk. Phototherapy and platelet-rich plasma (PRP) also show potential but need further study. The complexity and heterogeneity of AA necessitate personalized treatment strategies based on biomarkers. Future therapies aim to restore immune privilege and achieve systemic immune tolerance, potentially transforming AA into a manageable chronic condition. Despite promising developments, challenges like regulatory approvals, high costs, and lack of long-term safety data remain. A comprehensive framework for patient stratification is needed to enhance treatment efficacy and adherence.
